- Refuse or suspend your access to the National Immunisation Program (NIP) and State-funded vaccines, and
- Revoke your access to the NIP and State-funded vaccines
in NSW Health's absolute discretion, including where NSW Health
suspects that you are not complying with the requirements for the provision of vaccines.
- All information provided, including any information requested by NSW Health
is complete, accurate, and a true reflection of your site's vaccine-related practices and processes.
- You will submit relevant documents for cold chain review if requested by NSW Health.
Delays in providing this may result in suspended vaccine access, or revocation of your access,
until the information is received and reviewed.
- NSW Health (including all relevant public health units) may conduct audits and site visits
to monitor compliance with vaccine handling standards.
By signing in, you acknowledge and consent to the collection, use, and disclosure of your personal information, by the NSW Health with State Vaccine Centre as its authorised representatives, and relevant third parties, for the purpose of delivering communications in the administration of the immunisation program. The provider agrees to receiving such communications which may include, but are not limited to:
- information on changes to the national or state immunisation programs
- notifications about National Immunisation Program Schedule amendments
- updates regarding the process for ordering vaccines
- updates regarding other operational changes
- any other communications necessary for the effective implementation and management of the national and state immunisation programs.
